| Handicapped Persons Archery Permit** Present a completed Handicapped Persons Archery Hunting Permit [PDF, 69 KB] form signed by a physician at any IDFG Regional office. Physician certifies that applicant has a permanent disability whereby they do not have use of one (1) or both of his arms or hands and the applicant is capable of holding and firing, without assistance from other persons, a crossbow per Idaho Code. |

| Disabled Persons Motor Vehicle Permit** Present a completeed Disabled Persons Motor Vehicle Hunting Permit [PDF, 290 KB] form signed by a physician at any IDFG Regional office. Physician certifies the applicant is handicapped and has severe mobility impairment per Idaho Code. The words "permanently disabled" on reverse side of Idaho Drivers license is acceptable in lieu of physician certification form. | Disabled American Veterans Combination Hunting and Fishing License Letter from Veterans Affairs office showing a percentage of disability, letter or statement from Veterans Affairs office showing individual is nonservice-connected pension or a service-connected benefit with at least 40% disability. |

| Institutional Fishing Permit Present completed Institutional Fishing Permit [PDF, 99 KB] form at any IDFG Region office. Each inmate of the institution must be issued a permit. This permit is only available to inmates of the State Hospital North, State Hospital South, Idaho State School and Hospital, and state Veterans homes. |
